FAQs regarding this service
The GP Self-Referral service allows you to request specialist referrals, imaging tests (including X-ray, ultrasound, CT or MRI), and blood tests without needing a video consultation with a GP. You simply complete a secure online questionnaire, which is clinically reviewed by a GP to ensure your request is appropriate and safe.
Select the Self-Referral Questionnaire, which will branch you to the relevant request, complete all sections as accurately as possible, and submit your request. A GP will review your information and confirm whether the referral or test can be arranged.
- Self-referral questionnaires are designed for patients who know what test or referral they need.
- If you prefer to use a video-based review – please book using the self-referral video page
- If the GP reviewing your questionnaire needs more information, or if your request is not suitable for self-referral, you may be contacted to arrange a video GP consultation.
You can self-refer for:
- Imaging tests: X-ray, ultrasound, CT, MRI etc
- Specialist referrals
- Blood tests
All requests are clinically assessed to ensure they are safe and appropriate.
Your questionnaire is reviewed by a GP. One of the following will happen:

Yes. Every self-referral questionnaire is clinically reviewed by a GP to ensure safety, accuracy, and appropriateness. This is a key part of our clinical governance process.
The self-referral questionnaire is for referrals only. If treatment or a prescription is required, this will need to be arranged through a video-based GP consultation, where a doctor can safely assess your condition and recommend appropriate treatment.
