Gym or Health Club Membership Cancellation Letter

In the event of illness, injury or a health concern that hinders your ability to work out, a medical document may be necessary to pause or terminate your health club or gym membership.

Upon receiving evidence & carrying out a consultation with you, our medical professionals can issue a termination certificate. This certificate will verify your health condition and recommend that a pause or termination of your membership is advisable.

How It Works

At Dr.Online, we aim to make the process of obtaining a Medical letter as straightforward and convenient as possible. Here’s how it works:
  1. Select your letter: Select the letter your require, add to cart & make payment.
  2. Fill in the questionnaire & upload your evidence: Once your appointment is confirmed, you will receive a questionnaire via email. You will need to provide your name, date of birth, email address, details of your medical issue and a photo ID.
  3. Attend the video consultation: Attend the video consultation at your selected time to discuss your medical issue and condition with one of our GPs.
  4. Receive your letter: At Dr.Online, we will provide you with a letter following discussion with one of our doctors. This will verify your ailment and symptoms, denote whether your condition is persistent and specify any reasons for absence, recommendations or mitigating circumstances.

What do I need to provide?

  • Photo ID – passport or driving license
  • Details of when your condition began
  • Whether it’s ongoing or you’re now recovered
  • Upload of your digital summary care record for supporting evidence
  • Details of your usual/NHS GP
  • Completed medical questionnaire

For security purposes the following information is required to be provided via the Pre-Consultation Questionnaire/during the Video Consultation

  • Photographic ID
  • Name of Workplace and Job Role
  • Your NHS GP details for correspondance

If you are unable to work or require amended duties your employer may request a Fit Note completed by a medical professional. Fit notes are sometimes referred to as a sick note or doctor’s note. 

For the first 7 days you are able to complete a self certification document via the Government website (free of charge) but if you would prefer a private note we can facilitate this.

Our Fit Notes contain all the information needed by your employer or institution, including details of the issuing GP and their GMC (General Medical Council) number, reason for your sickness/amended duties and duration.

They also include a verification bar code which your employer can use to ensure it is a valid certificate.

 

  • We provide Private Fit Notes, these cannot be used for Department of Work & Pension purposes such as benefits. 
  • They are for a maximum duration of 14 days, and we cannot backdated from the consultation date. If extensions required you are required to book a further consultation to discuss this with one of our GPs. Maximum total duration we can issue is 28 days.
  • We are required to send a copy of the consultation to your regular NHS GP for continuity/information sharing purposes.
  • Your employer is not duty bound to accept these Fit notes, it is at their discretion.
