Self-Certification Sickness Certificate

For Adults (18+)

Same-day self-certification medical certificate to confirm that you have been unwell or sick within the first 7 days of an illness.

If you have been ill or have a health condition that has affected your ability to work, you can self-certify within the first 7 days of illness.
Our same-day self-certification medical certificate will detail your illness and symptoms, whether your condition is ongoing or resolved, and confirm both the start and end dates of your self-certification.
No appointment is necessary – simply apply online. You will receive your certificate via email.
Please note: This is a private UK doctor-led certificate service. The medical certificates we provide are not the same as NHS Med3 Fit Notes, which are only available from healthcare professionals delivering NHS services.
You are not legally required to provide a Med3 fit note or work sickness certificate to your employer within the first 7 days of illness, although many employers request written confirmation. Always check your employment contract.

What will I need to supply?

To enable us to assess your condition and issue a medical certificate, we will need the following information:
  • A photo of your passport, driving licence or workplace ID
  • First and last day of sickness self-certification
  • Whether your sickness is ongoing or you have now recovered
  • The name of your workplace

What does self-certification mean?

If you are absent from work due to illness for 7 days or less, your employer typically shouldn’t request medical proof of your sickness. Instead, they may require you to verify your illness yourself. This can be done by completing a form upon your return to work, a process known as self-certification.

What is the difference between a sick note and a self-certification?
A sick note details your health condition and ability to work and is issued by a UK Doctor after an assessment. Self-certification is a certificate that allows you to certify a brief illness, up to 7 days, without the need for a consultation with a doctor.
